5. Transfer Your Funding

Call My Aged Care on 1800 200 422 to re-activate your referral code, then provide it to Lumia Care. Since Services Australia manages HCP funds, your previous provider will finalise your account, and the funds will be transferred to Lumia Care within 70 days. We’re here to oversee this process for you, ensuring a hassle-free transition.

 

6. Confirm Transition Date and Start Your New Care!
