3. Inform Your Current Provider
When you’ve decided to make the switch, notify your current provider. Giving formal notice ensures continuity of care during the transition. Thanks to recent regulations, switching providers no longer involves exit or joining fees, so you won’t face unexpected costs.
4. Contact Your New Provider
Once you’ve chosen Lumia Care, we’ll help you craft a personalised care plan and handle the required documentation. Remember, you have 56 days from the agreed end date with your current provider to engage a new one, and Lumia Care will make sure everything is sorted efficiently.
5. Transfer Your Funding
Call My Aged Care on 1800 200 422 to re-activate your referral code, then provide it to Lumia Care. Since Services Australia manages HCP funds, your previous provider will finalise your account, and the funds will be transferred to Lumia Care within 70 days. We’re here to oversee this process for you, ensuring a hassle-free transition.
6. Confirm Transition Date and Start Your New Care!
Coordinate the end date with your current provider and the start date with Lumia Care. This ensures no interruptions in your care services. Once everything is confirmed, you can enjoy high-quality, personalised care with us.
