What Is the Support at Home Program?

On 1 November 2025, the Australian Government replaced the Home Care Packages (HCP) and Short-Term Restorative Care (STRC) programs with a single, streamlined system called Support at Home. 

This new program is designed to simplify access to aged care services and make it easier for older Australians to receive the right support when and where they need it.

What Services Are Included in the Support at Home Program? 

The Support at Home program offers a clear and structured list of in-home care services, grouped into three core categories. These services are designed to empower older Australians to live safely, comfortably, and independently in their own homes. Support at Home offers a structured list of services to help older Australians remain safe, independent, and supported in their own homes. 




Short-Term Care Options

When your needs change quickly, these care options provide targeted support. 

Restorative Care Pathway 

This short-term support program is designed to help you recover after a health setback such as surgery, illness, or injury. The goal is to improve your strength, confidence, and mobility so you can remain independent at home and avoid further hospitalisation. 

You’ll receive tailored allied health services for up to 12 weeks, with the possibility of a 4-week extension depending on your circumstances. 

End-of-Life Pathway 

For individuals with a terminal diagnosis or a life expectancy of around three months or less, this pathway provides additional funding and compassionate support. It allows you to remain in a familiar and comfortable home environment, surrounded by loved ones, with services focused on comfort, dignity, and quality of life. 

Up to $25,000 in additional support is available for compassionate, palliative in-home care for up to 16 weeks. This funding ensures dignity, comfort, and personalised care during the final stages of life.
