Assessments are arranged through My Aged Care, the Australian Government’s entry point for aged care assistance. In most cases, the assessment takes place in your home, where you may feel more comfortable discussing your needs and the type of support that could help you continue living life on your terms.
For many people, the word “assessment” can feel a little daunting and may even sound like a test. In reality, an assessment for aged care is simply a respectful conversation focused on exploring services that may help maintain your independence and wellbeing. It does not automatically lead to residential care or a loss of control over your decisions.
What happens during the aged care assessment?
Here's what you can generally expect during the assessment:
Understanding your eligibility
One of the first parts of the assessment involves confirming your eligibility for government-funded aged care services. The assessor may ask about your health, mobility and how you currently manage everyday activities. This helps build a clearer picture of your situation and the level of care that may be appropriate.
A conversation in your home environment
Assessments are typically arranged in your home, helping the assessor understand your living situation and daily routines. If you prefer, a family member, friend or carer can be there with you during the meeting for added support.
Topics the assessor may discuss
The assessor may ask questions about different aspects of your daily life, including:
Your current health and any medical conditions
How you manage everyday activities such as cooking or personal care
The support you currently receive from family, friends or other services
Any difficulties you experience that may affect your independence or safety at home
Determining appropriate services
Based on the information you share, the assessor may recommend one or more of these services:

How do I arrange my aged care assessment?
You can apply for an aged care assessment service yourself or ask a healthcare professional such as your GP or hospital social worker to help arrange the referral. There are three main ways to begin the application process:
Online
You can submit your application through the My Aged Care website. The online form usually takes around 15 to 20 minutes to complete.
By phone
If you’re more comfortable speaking to someone directly, you can call My Aged Care on 1800 200 422. They can answer any questions and help lodge your application over the phone. You are welcome to have a carer, relative or friend join the call.
In person
If you prefer an in-person conversation, you can arrange a meeting with an Aged Care Specialist Officer (ACSO) at a Services Australia centre.

How to prepare for an assessment for aged care
Before the assessment, it can be helpful to reflect on the support you currently receive and where additional assistance may be required. You may also be asked to have a few documents and details ready, including:
Your Medicare card and another form of identification, such as a driver’s licence, healthcare card, passport or DVA card
Any referrals or notes from your doctor
Contact details for your GP or other healthcare professionals involved in your care
Any details or questions you may have about aged care services that you would like to discuss
