Seamless Coordination with Hospitals
We work closely with both public and private hospital systems through various partnerships and agreements, ensuring seamless care whether you’re transitioning home from a hospital stay or require ongoing wound management.
Our team can also work directly with your GP, specialist or healthcare team to support continuity of care and keep your wound care plan aligned with your treatment goals and recovery needs. For patients requiring post surgical wound care, we help coordinate services after discharge, including dressing changes, monitoring and implementation of a personalised care plan for surgical wounds where required.
To make the process even easier, a specialist nurse will visit when required to assist with onboarding and complete the referral process, so you can start receiving care without delay.

Depending on your treatment plan, this support may also extend to services such as IV medication administration at home, allowing prescribed therapies to continue without the need for a prolonged hospital stay.
