What is an End of Life Pathway? Everything You Need To Know

When someone you love is nearing the end of their life, every moment matters. For many older Australians and their families, the hope is simple: to stay at home, in familiar surroundings, with the people they love — and with care that brings comfort, not complication. 

That’s why the Australian Government introduced the End of Life Pathway on 1 November 2025, as part of the new Support at Home program.

This new pathway offers fast, extra support to help people spend their final weeks where they want to be — at home, surrounded by peace, love, and dignity. 

What Is the End of Life Pathway? 

The End of Life Pathway is a short-term care option designed for people who have been told they have three months or less to live. It provides up to $25,000 in government-funded care over 12 weeks, with the option to extend to 16 weeks if needed. 

Most importantly, it helps families act quickly — without the stress of waiting lists or complicated processes. 

What Is the Purpose of End of Life Care Pathway? 

The purpose is simple but powerful: to provide care that’s focused on comfort, dignity, and time together. It helps take the pressure off families, offering practical support and compassionate care when it’s needed most — whether that’s help around the house, personal care, or nursing. 

This care is not just about what’s needed medically. It’s about helping someone feel safe, respected, and at peace in their final weeks. 

Who Can Access the End of Life Pathway? 

If a loved one has a prognosis of three months or less, they may be eligible. A doctor or nurse practitioner will need to complete a government form confirming their eligibility. This is then submitted to My Aged Care to trigger a fast-tracked aged care assessment.
