Cinical Manager (Community Nursing Program) - Dulwich
About the Role:
Dulwich - We are seeking an experienced and dedicated Community Nursing Program Clinical Manager to lead and oversee our community nursing initiatives. This role will be responsible for the development, implementation and management of our community-based healthcare programs including Hospital in the Home, NDIS, Private, Brokerage and VHC clients, ensuring the highest standards of patient care and service delivery are maintained. This role will require strong leadership, program management and clinical experts to drive health outcomes within the community.
Develop, implement and manage the community nursing programs.
Supervise and support a team of community nurses, providing leadership, training and performance evaluations.
Deliver outstanding customer service to our clients through effective complaint resolution, problem-solving, and offering sound clinical guidance.
Coordinating NDIS assessments.
Monitor complaints, incidents and deteriorations.
Triage and monitor referrals.
Provide education and upskilling for registered and enrolled nurses when applicable.

To be successful in this role you will possess a Bachelor of Nursing with acute or community nursing experience. We are seeking candidates who have excellent written and verbal communication skills, strong critical thinking, excellent clinical skills and ability to lead a team of enrolled nurses to deliver exceptional quality services.
Bachelor of Nursing and current APHRA registration.
Proven experience in managing a clinical team.
Strong attention to detail with the ability to think critically.
Strong leadership, organisational and communication skills.
Knowledge of current health policies, community healthcare strategies and best practice nursing.
Ability to build strong relationships with key stakeholders.
