What Is Community Nursing?
The Community Nursing (CN) Program provides clinically necessary nursing and personal care services in the comfort of your own home. These services are delivered by qualified nurses and support staff to help you maintain your health and independence.
Services may include:
✔ Medication management — Keeping track of medications can become challenging, especially when treatment plans change or involve several prescriptions. Our nurses can assist with organising, administering and monitoring medications to ensure they are taken safely and as needed.
✔ Wound care — Professional assessment and treatment of wounds, including dressing changes, infection prevention and monitoring the healing process. Timely and consistent care helps support proper healing and may reduce the risk of complications or hospital visits.
✔ Hygiene assistance (showering, dressing, grooming) — Help with daily personal care tasks such as showering, dressing, grooming and maintaining personal hygiene. Our team provides this support in a way that respects your independence while helping you maintain your daily routine.
✔ Palliative care and comfort support — Compassionate care for veterans with life-limiting conditions, focusing on comfort, dignity and quality of life at home. This care also provides reassurance for families, knowing their loved one is receiving attentive and supportive care.
✔ Chronic disease management — Ongoing nursing support for managing long-term health conditions, including monitoring symptoms, supporting treatment plans and helping prevent complications. Regular check-ins help identify changes early so care can be adjusted when needed.

Who’s Eligible?
There is no cost to eligible Veterans, War Widows, or Widowers receiving care under the Department of Veterans’ Affairs (DVA) Community Nursing (CN) Program.
You may qualify for fully funded in-home DVA Community Nursing services if you have one of the following:
✔ Veteran Gold Card – Covers all clinically necessary community nursing and personal care services.
✔ Veteran White Card – Covers nursing services only if the care is required due to an accepted service-related condition. If you're unsure about your eligibility, we can assist in checking with DVA.

Getting Started: How to Arrange Care
Arranging an Assessment
Getting started is simple! All you need to do is visit your local doctor and discuss a referral for Community Nursing assistance. Let them know you’d like Lumia Care to provide your Veterans' home health care services, and they will send the referral directly to our Care Services team.
Referrals can also be made by:
✔ Your Local Medical Officer (LMO) or GP
✔ A treating doctor in a hospital
✔ A hospital discharge planner
✔ A Nurse Practitioner specialising in community nursing
✔ A Veterans’ Home Care (VHC) Assessment Agency
Once your referral is sent, Lumia Care will take care of the rest!

Creating Your Personalised Nursing Care Plan
Once you are approved for Veteran Community Nursing services, your Lumia Care team will guide you in developing a personalised nursing care plan. If you would like, a family member or carer can also be involved in this process, ensuring your needs and goals are taken into account.
Your nursing care plan outlines the care and support you will receive at home. This may include:
✔ The nursing care activities identified during your assessment
✔ Your personal goals and expected outcomes from the care provided
✔ The agreed days and approximate times when nursing visits will take place
Before DVA community Nursing services begin, you will be asked to review and sign the care plan to confirm that you agree with the proposed care. You will receive a copy for your records. If you are unable to sign the document yourself, a family member or carer can sign on your behalf.
As your health needs may change over time, your nurse will regularly review your care plan and make updates when needed. This ensures your care continues to support your wellbeing and independence at home.
