Check Your Eligibility
To apply for Support at Home (transitioned from Home Care Package), you must:
✔ Be aged 65 or over (or 50+ for Aboriginal and Torres Strait Islander people)
✔ Need support to continue living independently at home
Start by contacting My Aged Care at myagedcare.gov.au or call 1800 200 422 to check your eligibility and register for an assessment.
