What Is A Home Care Package?

As part of the Australian Government’s aged care reform, the Support at Home program has now replaced the Home Care Package (HCP) system. While the program structure has changed, the same types of services — including personal care, nursing, domestic help, transport, and allied health — continue to be supported. Importantly, people who previously received a Home Care Package will not be worse off under the new system.
Originally, HCPs provided older Australians with flexible funding to stay safe and independent at home, rather than entering residential aged care. That purpose continues under Support at Home, with improvements to how care is delivered, coordinated, and adjusted over time.

How Do I Aquire for a Home Care Package?

Check Your Eligibility
To apply for Support at Home (transitioned from Home Care Package), you must:
✔ Be aged 65 or over (or 50+ for Aboriginal and Torres Strait Islander people)
✔ Need support to continue living independently at home
Start by contacting My Aged Care at myagedcare.gov.au or call 1800 200 422 to check your eligibility and register for an assessment.
Complete a Home Assessment
A member of your local Aged Care Assessment Team (ACAT) will visit you at home to assess your needs. This determines your eligibility and what level of care funding you may receive.
Receive Your Package and Choose a Provider
If approved, you’ll be allocated Government support based on your assessed needs. You can then choose a trusted provider — like Lumia Care — to manage and deliver your in-home services.
Create a Personalised Care Plan
