How Do I acquire a Home Care Package?

Check Your Eligibility

To receive a government-subsidised Home Care Package, you must: 
Be 65 years or older (50+ for Aboriginal and Torres Strait Islander people). 
Need support to continue living independently at home. 

Eligibility is assessed through My Aged Care, which determines if you require basic, low, intermediate, or high-level care. 

Not sure if you qualify? You can check your eligibility and apply through My Aged Care or call 1800 200 422. 

Complete an Assessment

A member of the Aged Care Assessment Team (ACAT) will visit you at home to assess your care needs and recommend a Home Care Package level. 

Receive Your Package and Choose a Provider

Once approved, you will be assigned a Home Care Package level and can choose a trusted provider like Lumia Care to manage your care.
