Support at Home is Australia’s in-home aged care program, now fully in place across the country. It brings together services like help around the house, personal care, and nursing or allied health — all designed to support older people at home, in a way that fits their life. 

Whether you're arranging care for yourself, a parent, or someone you support, it's about getting the right help when it's needed — tailored to your needs, goals, and home environment. 

What kind of support can I get at home?

When you're approved for a Support at Home package, your services are tailored to what matters most — staying safe, well, and connected at home. Depending on your needs, support may fall into one or more of these three areas:

Everyday Living Services 

This support helps with daily tasks that keep your home safe, clean and manageable. It can include general house cleaning, laundry, grocery shopping, and preparing or delivering meals. For some clients, it also involves light gardening or basic home maintenance — just enough to stay on top of the little things without stress.

Helen lives alone with arthritis. With weekly help for groceries, laundry and meal preparation, she’s able to stay comfortable at home and maintain her independence without relying on family. 

Independence Support Services

These services focus on your wellbeing, mobility, and connection to everyday life. You might receive support with showering, dressing or grooming, help to get out and about safely, or someone to check in regularly. It can also include medication reminders, transport to appointments, or simply having someone to talk to.

Jim receives daily support with personal care and transport. His regular morning visits help him feel more confident and allow him to stay involved in his community without over-relying on family members. 

Clinical Support Services

These services bring nursing and allied health care directly into your home. Depending on your needs, this may include help with wound care, managing medications, monitoring chronic conditions like diabetes, or post-hospital recovery support. Allied health services — like physiotherapy, podiatry or occupational therapy — can also be part of your ongoing care.

Fatima lives with diabetes and mobility limitations. Her care plan includes nursing check-ins, podiatry for foot health, and physiotherapy to help her stay active at home without needing frequent clinic visits.

Short-Term Pathways

Support at Home also includes focused care programs for people recovering from a setback or managing life’s final chapter. These are designed to provide additional care when it's needed most. 

Restorative Care Pathway

Restorative care offers a short-term plan to help you recover and regain confidence after an illness, injury or hospital stay. Services may include physiotherapy, occupational therapy, home equipment, and in-home support — typically for six to twelve weeks, with a clear focus on reablement.

Roslyn received restorative care following surgery. With support from a physiotherapist and short-term assistance at home, she was able to build strength and return to independent living without additional care. 

End-of-Life Pathway

When someone is nearing the final stage of life, this pathway provides the compassionate support needed to remain at home. It is designed for people who may have only a short time left, often a few months or less and wish to be cared for in a familiar place, with family, carers and loved ones close by. Services can include personal care, domestic help, clinical and allied health support, assistance accessing equipment and aids, emotional comfort, and respite for family and carers. Our goal is to provide peaceful and supportive care when it is needed most.
