Registered Nurse - Office Based Role
About the Role:
Adelaide - The Registered Nurse – Community Nursing Programs is an office-based position that will assist in the delivery of in-home care for NDIS, Private and Brokerage and DVA clients. Key to the role is working with a team of Registered Nurse and Enrolled Nurses in case coordination through Pop-Up Health's software platform, Sandwai. Communicating clearly and transparently with the team, clients and their families you will imbed trust and cooperation.
Suitable candidates will be available to work shifts across a 7-day roster.

Key Duties & Responsibilities:
Packing wound boxes, unpacking stock and general maintenance of the consumables in the wound room.
Providing general customer service by answering phone calls, complaint mitigation, problem solving, providing clinical advice within scope and escalating when required to Clinical Manager/Team Leader.
Completing road shifts as required.
Monitor complaints, incidents, and deteriorations in Sandwai with the wider team and ensure they are responded to in a timely manner – escalate as required to Clinical Manager.
Complete client assessments as required.
Provide clinical advice to road employees as required.
Triage and monitor referrals through the referral inbox ensuring client onboarding documentation is completed in a timely and accurate manner.
Create client service plans for home visits, develop an appropriate care plan and prepare documentation for initial visit.
Liaise with existing clients and monitor feedback.
Support field nurses with any queries or concerns.
Experience:
3-5 years' experience as a Registered Nurse.
Community nursing experience (desired).
Knowledge of Aged Care/NDIS Standards.
Skills & Knowledge:
Competent computer skills.
Excellent clinical and risk assessment skills.
Ability to recognise deterioration in areas of cognition, mobility, nutrition etc and implement strategies to re-enable client level of function to baseline.
Ability to work in a fast-paced environment.
Ability to work autonomously and within a team.
Demonstrate initiative and problem solving as required.
Ability to have conflict resolution skills.
Strong organisation and time management skills.
Requirements:
Bachelor of Nursing and current AHPRA Registration
National Police Clearance, DHS Clearances (NDIS, WWCC)
First Aid, CPR, Manual Handling
Current Driver’s Licence
